So, what is a dental implant?
Ask anyone who has lost a tooth and they will tell you the same thing: it is never really about the tooth.
It is about the photograph you would rather not be in. It is about ordering something softer than you actually wanted at The Bulls Head because you are not confident chewing on that side. It is about the small, permanent adjustment you have made to how you smile, which you stopped noticing years ago but everyone in your family did.
Nobody comes here because a tooth is missing. They come because of what the missing tooth has quietly taken with it.
A dental implant gives that back. It is a small titanium post placed into the jawbone, sitting precisely where the root of your tooth used to be. Over the following months the bone grows onto it and grips it as firmly as the original root did. A crown, bridge or denture is then fixed on top.
The result behaves like a natural tooth. You bite with it, chew with it and clean it like the rest of your teeth. Nothing comes out at night and nothing shifts when you eat.
Why the root matters more than people expect
Implants are the only tooth replacement that also replaces the root, and that is not a technicality.
When a tooth goes, the bone that held it stops being stimulated and starts to shrink. Over years that changes the shape of the lower face, makes dentures fit progressively worse, and makes any future treatment more complicated and more expensive. An implant keeps that bone working and stops the process where it stands.
Titanium is used because bone fuses to it naturally, a process called osseointegration. It is the same material used in hip and knee replacements, and it has been used in dentistry for over fifty years. None of this is new or experimental.

Am I a suitable candidate?
Most adults missing one or more teeth are suitable for implant treatment. Dr Nikhil will assess your case properly at your consultation, but as a general guide:
Usually suitable if you
- Have one or more missing teeth, or teeth that cannot be saved
- Have healthy gums, or gum disease that can be treated first
- Have sufficient jawbone, or bone that can be rebuilt through grafting
- Are in reasonable general health
- Do not smoke, or are willing to stop during healing
We will need to assess further if you
- Have significant bone loss, since grafting, a sinus lift or zygomatic implants may be required
- Have untreated gum disease, which must be resolved before implants are placed
- Have diabetes, which affects healing and needs to be well controlled
- Smoke, since this increases the risk of implant failure considerably
- Take bisphosphonates or certain other medications affecting bone
Implant treatment is contraindicated during pregnancy. We can plan your treatment and begin once it is appropriate to do so.
If you have been told elsewhere that you are not suitable
It is worth a second opinion, and we say that having seen it go the other way many times.
Most people who are turned away are told there is not enough bone. That is frequently accurate as far as conventional implants go, and it is rarely the end of the matter. Bone can often be rebuilt. Angled placement techniques engage bone that is still there. Zygomatic implants anchor into the cheekbone where the jaw has nothing left to offer.

The scan shows bone volume, density, nerve position and sinus anatomy in detail a standard x-ray cannot. This is what makes accurate planning possible rather than educated guesswork.

2Step two: your treatment planYou receive a written plan setting out the number of implants required, the timeline, each stage of treatment and every cost. Finance options are explained at the same time. Nothing proceeds until you have agreed to all of it.

3Step three: any preparatory treatmentIf gum disease needs treating, a tooth needs removing or bone needs rebuilding through grafting or a sinus lift, that happens first. Healing time varies depending on what is required.

4Step four: implant placementThe implant is placed under local anaesthetic. Most patients describe feeling pressure rather than pain, and for a single implant the procedure usually takes under an hour. You go home the same day. Sedation is available if you would prefer it.

5Step five: healing and integrationThe implant needs three to six months to fuse with the bone. You will have a temporary tooth during this period so there is no visible gap. Some cases are suitable for immediate loading, where teeth are fitted the same day.

6Step six: fitting your final teethOnce integration is confirmed, the final crown, bridge or denture is made and fitted. Shade and shape are matched to your surrounding teeth.

Implant treatment is priced per case, because the number of implants, the condition of your bone and the type of restoration all affect the cost.
A single implant is quoted in two stages, since the placement and the crown happen several months apart. Bone grafting and sinus procedures are only required where the scan shows insufficient bone, and not every patient needs them.
You will receive full written costs before any treatment begins.

Croft Dental sits at 64 Manchester Road, on the Handforth side of Wilmslow town centre. Coming up from The Carrs, we are north east along Manchester Road. If you reach Handforth Dean, you have gone too far.
We are around ten minutes on foot from Wilmslow station, five minutes from Handforth, and roughly ten minutes from Alderley Edge up the A34.
Patients travel to us for implant treatment from Alderley Edge, Handforth, Styal, Prestbury, Mottram St Andrew, Knutsford, Macclesfield, Poynton, Mobberley, Chelford, Bramhall, Cheadle and Hale, as well as from South Manchester along the A538 from the airport. For zygomatic cases, people come considerably further than that, simply because so few practices offer it.

Dr Nikhil Oberai

Dr Nikhil leads implant treatment at Croft Dental. He has placed hundreds of implants, from single tooth replacement through to full arch reconstruction and zygomatic cases, with a success rate of 99.5%.
His Master’s degree from the University of Manchester involved several years of postgraduate study and supervised clinical practice devoted entirely to implantology. Many dentists placing implants have completed shorter certificate courses instead. Both are perfectly legal. The depth of training differs, and it is the question patients ask least and should ask most.
He is a member of the Association of Dental Implantology, a Key Opinion Leader for Osstem, and an Honorary Clinical Teaching Fellow at the University of Manchester, where he helps train dentists on the implant programme. Practices across the North West refer their complex implant cases to him.

Dental implants are designed as a permanent tooth replacement. With good oral hygiene and regular dental visits, implants frequently last for decades. The crown fitted on top may need replacing after fifteen to twenty years of normal wear, in the same way a natural tooth may eventually need a new crown.
Implant placement is carried out under local anaesthetic and most patients report feeling pressure rather than pain during the procedure. Some tenderness and swelling in the days afterwards is normal and usually manageable with ordinary painkillers. Sedation is available from £350 for patients who would prefer it.
In many cases, yes. Bone can often be rebuilt through bone grafting from £450 or a sinus lift from £850, carried out before the implant is placed. Where the upper jaw has severe bone loss and conventional implants are not possible, zygomatic implants anchor into the cheekbone instead. A CBCT 3D scan determines what is possible in an individual case.
Most single implant cases take between three and six months from placement to final crown, allowing time for the implant to integrate with the jawbone. Some cases are suitable for immediate loading, where temporary teeth are fitted on the day of surgery. Cases requiring bone grafting take longer, since the graft must heal before the implant can be placed.
The number depends on how many teeth are missing and where. One missing tooth requires one implant. Two or three adjacent teeth can often be replaced with a bridge supported on two implants rather than one implant per tooth. A full arch typically uses four to six implants supporting a fixed bridge. The number is determined from a CBCT 3D scan.
There is no upper age limit for dental implants. Suitability is determined by general health and bone quality rather than age, and patients in their seventies and eighties are treated routinely. The minimum age is around eighteen, since implants should not be placed until the jaw has finished growing.
No. Where the implant site is visible when smiling, a temporary tooth or bridge is provided so there is no visible gap at any stage. For full arch cases, a fixed temporary bridge is usually fitted on the day of surgery.
