Full arch implant treatment replaces an entire upper or lower set of teeth using a small number of dental implants to support a fixed bridge. With Smile in a Day, the implants are placed and a fixed set of teeth attached during the same appointment, so patients leave with a full arch rather than a gap. At Croft Dental on Manchester Road in Wilmslow, full arch treatment is carried out by Dr Nikhil Oberai, who holds an MSc in Implant Dentistry from the University of Manchester and has a 99.5% success rate across hundreds of cases. Packages start from £11,500 per arch, typically using four to six implants. The definitive bridge is fitted four to six months later once the implants have fused with the bone. Consultations are free of charge.

So, what is Smile in a Day?

It is exactly what the name suggests, and it is worth taking a moment over because most people assume it cannot be true.

You arrive in the morning with failing teeth, or a denture you have grown to resent. You leave that afternoon with a fixed arch of teeth secured to implants. Nothing comes out. Nothing needs adhesive. There is no long stretch of walking around with a gap, or wearing a temporary denture, waiting for something to heal.

For anyone who has spent years managing a mouth rather than simply having one, that is a considerable thing.

The honest detail

The teeth fitted on the day are a functional temporary bridge. They look good and they work properly, but they are not the final result. Once the implants have fused with the bone over the following months, the temporary is replaced with a definitive bridge made from stronger material and finished to a higher standard.

Why it is possible at all

Implants only need to be left undisturbed if they are unstable when placed. Where your scan shows good bone and the implants achieve sufficient stability during surgery, they can carry a temporary bridge straight away. This is called immediate loading.

And why it is not always possible

Dr Nikhil will only fit teeth on the day if the implants are stable enough to take them. If they are not, we protect the outcome rather than the timeline, and you wear a temporary denture for a short period instead.

Your scan gives a strong indication beforehand and this is discussed with you well before surgery, so nobody is surprised on the day. But the final judgement is made in the chair, and we would rather tell you that now than promise something we cannot guarantee.

All-on-4

Four implants supporting a full fixed arch. The most widely used full arch protocol and suitable for the majority of patients. Angling the rear implants means bone grafting can often be avoided altogether.

Which one is right for you

Decide after your CBCT 3D scan, not before.
Dr Nikhil will explain what your bone allows, what each option costs, and which he would recommend and why. If an implant retained denture would give you most of what you want for considerably less, he will say so.

Am I a suitable candidate?

Full arch treatment generally suits patients in one of these situations:

  • You wear a full denture and want something fixed. Movement, sore spots, adhesive and the foods you have quietly stopped ordering are the reasons most denture wearers come to us.
  • Most of the teeth in one jaw are missing or failing. Where only a handful remain and they are not reliable, replacing the arch is often more predictable than treating each tooth individually.
  • Advanced gum disease has loosened your teeth. Where the supporting bone has deteriorated across the arch, a full arch solution addresses the whole problem rather than delaying the inevitable.
  • You have had years of repeated dental work. Some patients reach a point where they would rather resolve the whole situation than keep treating one tooth at a time. That is a perfectly legitimate reason.

 

  • Dr Nikhil will assess your case properly with a CBCT 3D scan, but broadly:

Usually suitable if you

  • Are missing all or most of the teeth in one or both jaws
  • Have failing teeth that cannot reliably be saved
  • Have bone available at the front of the jaw, which most patients do
  • Are in reasonable general health
  • Do not smoke, or are willing to stop while the implants heal
  •  

We will need to assess further if you

    • Have significant bone loss, since more implants, grafting or zygomatic implants may be recommended
    • Have uncontrolled diabetes, which affects healing
    • Smoke, as this raises the risk of implant failure considerably
    • Take bisphosphonates or other medications affecting bone
    • Grind your teeth heavily, which may influence the number of implants and the bridge material

    Implant treatment is contraindicated during pregnancy. We can plan your case and begin once it is appropriate.

If you have been told there is not enough bone

This is the most common reason people are turned away, and it is frequently not the end of the matter.

Angled placement engages bone that is still there rather than requiring bone to be built where it has gone. Where the upper jaw genuinely has too little for any conventional approach, zygomatic implants anchor into the cheekbone instead.

Both are carried out here. If you were told no at a practice offering neither, you were told what that practice could do rather than what is possible.

    Step one: consultation and 3D scan
    Dr Nikhil examines your mouth, discusses what you want to achieve and takes a CBCT 3D scan mapping bone volume, density and anatomy across the arch. Photographs and digital scans are taken at the same appointment.

    Step two: your treatment plan
    You receive a written plan setting out the number of implants recommended, whether same day teeth are likely in your case, the full timeline and every cost. Finance options are explained at the same time. Nothing proceeds until you have agreed.

    Step three: digital planning and design
    Your case is planned in three dimensions before surgery. Implant positions are determined from the scan, and the shape, size and shade of your new teeth are designed in advance. You see the design and can comment before anything is made.

    Step four: surgery day
    Any remaining teeth in the arch are removed and the implants placed under local anaesthetic, with sedation available if you would prefer it. Surgery for one arch typically takes two to three hours.

    Where implant stability allows, your fixed temporary bridge is attached before you leave. You go home with a full arch of teeth.

    The implants need four to six months to fuse with the bone. You wear the temporary bridge throughout and keep to a softer diet initially. Review appointments monitor your progress.

    Once integration is confirmed, the definitive bridge is made and fitted. Stronger material, more refined finish, with final shade and shape agreed with you.

    We review your implants and monitor them at regular appointments.

    Eating: once healed, most patients eat without restriction, including the foods denture wearers tend to avoid. During the temporary phase we ask you to keep to softer foods so the implants integrate undisturbed.

    Cleaning: your bridge is cleaned daily with a water flosser and interdental brushes to reach underneath. Straightforward once shown, though different from cleaning natural teeth.

    Speech: most patients adapt within a few days. Any initial difference settles as the tongue adjusts.

    Maintenance appointments: regular hygiene visits are required so the bridge can be cleaned professionally and the implants monitored. This genuinely matters, since implants can be lost through poor maintenance in the same way natural teeth are lost to gum disease.

    Longevity: the implants are designed to last decades. The bridge fitted on top will wear over time and may eventually need replacing, as any restoration would.

The three packages differ in the materials used and the specification of the final bridge, which affects appearance, strength and how long it lasts. Dr Nikhil will explain the practical differences at your consultation.

Your quoted fee covers the implants, the surgery, your temporary teeth and your final bridge. Everything is set out in writing before treatment begins, and there are no charges partway through that you were not told about.

Where to find us

Croft Dental sits at 64 Manchester Road, on the Handforth side of Wilmslow town centre. Coming up from The Carrs, we are north east along Manchester Road. If you reach Handforth Dean, you have gone too far.

We are around ten minutes on foot from Wilmslow station, five minutes from Handforth, and roughly ten minutes from Alderley Edge up the A34.

Patients travel to us for full arch and Smile in a Day treatment from Alderley Edge, Handforth, Styal, Prestbury, Mottram St Andrew, Knutsford, Macclesfield, Poynton, Bramhall, Cheadle and Hale, as well as from South Manchester along the A538 from the airport. For full arch work specifically, a good number come from considerably further across the North West.

Dr Nikhil carries out full arch and Smile in a Day treatment at Croft Dental, with a 99.5% success rate across hundreds of implant cases.

Full arch work is the most demanding implant treatment there is, and the depth of training matters more here than anywhere else. His Master’s degree from the University of Manchester involved several years of postgraduate study and supervised clinical practice devoted entirely to implantology.

He also places zygomatic implants for patients with severe upper jaw bone loss, a treatment offered by very few clinicians in the region, which means cases referred away elsewhere are handled here.

He is a member of the Association of Dental Implantology, a Key Opinion Leader for Osstem, and an Honorary Clinical Teaching Fellow at the University of Manchester, where he helps train dentists on the implant programme. Practices across the North West refer complex full arch cases to him.
